Reviewed by Russell Noga, Licensed Medicare Insurance Broker (all 50 states) — Last updated October 2, 2026

Aetna Medicare Advantage plans 2027 come from one of the largest Medicare Advantage carriers in the country, and one with some of the strongest quality scores. They also come from a carrier that is getting smaller for the second year in a row.

That combination makes Aetna worth a closer look before you renew or switch. Below is what is changing with Aetna for 2027, the plan types it sells, its star ratings, what its plans may include and how to compare them with other carriers.

Aetna Medicare Advantage Plans 2027 at a Glance

Aetna Medicare Advantage Figure Plan year
States with Aetna MAPD plans 43 states plus Washington, D.C. 2026
Medicare-eligible people with a $0 premium Aetna plan available About 82% 2026
Members in plans rated 4 stars or higher More than 81% 2026 ratings

Several of these figures are from 2026. Aetna’s 2027 plan-level details are now posting county by county on Medicare’s Plan Finder, and CMS releases 2027 star ratings in October. Confirm the premium, network and benefits on the specific Aetna plan offered in your county before you enroll. Not all plans offer all benefits.

Aetna Medicare Advantage 2027 changes: about 103 counties exited and broker commissions frozen on 123 plans in 33 states

What Is Changing With Aetna for 2027

Aetna, owned by CVS Health, is in its second straight year of pulling back. Going into 2026 it closed nearly 90 Medicare Advantage plans across 34 states, left about 100 counties on a net basis and exited one state entirely. For 2027, analyst reviews of the CMS landscape data show Aetna exiting about 103 more counties.

The bigger signal is a commission freeze. Aetna told marketing organizations it will not pay brokers to enroll new members in 123 Medicare Advantage plans across 33 states for 2027. Those plans are still on sale, but carriers often freeze commissions on plans they want to shrink, and a frozen plan has a higher chance of being closed in a later year.

IF YOUR AETNA PLAN IS ENDING

Non-renewal notices were dated October 2, 2026. If you get one, your plan stops on December 31. You can pick a new plan during the Annual Enrollment Period, October 15 to December 7, and you also get a Special Enrollment Period through the end of February 2027. If you do nothing, you return to Original Medicare on January 1, without drug coverage unless you choose a Part D plan.

Across the whole market, the news is steadier: CMS projects the average Medicare Advantage premium will fall to about $12 a month in 2027. See our guide to Medicare Advantage plans 2027 for the national picture.

Aetna Medicare Advantage Plan Types

  • HMO plans: use Aetna’s local network except in emergencies, usually with referrals for specialists. Often the lowest premiums, including $0 plans.
  • PPO plans: let you see out-of-network providers at a higher cost, usually without referrals. A better fit if you travel or split time between states.
  • Dual Eligible Special Needs Plans (D-SNP): for people with both Medicare and Medicaid.
  • Chronic Condition Special Needs Plans (C-SNP): for people with certain chronic conditions. Aetna expanded these to 18 states for 2026.

Aetna Medicare Advantage star ratings: more than 81% of members in 4-star or higher plans and 63% in 4.5-star plans

Aetna Medicare Advantage Star Ratings

Quality is Aetna’s strongest selling point. In the 2026 ratings, more than 81% of its Medicare Advantage members were in plans rated 4 stars or higher, and more than 63% were in 4.5-star plans. That is well above several other large carriers.

2027 ratings arrive in October. About half of the measurement cut points got harder for 2027, so many plans across the industry may lose ground. Check the 2027 rating of the specific Aetna contract sold in your county, not just Aetna’s average.

What Aetna Plans May Include in 2027

In 2026, every Aetna Medicare Advantage plan included dental, vision and hearing benefits and a SilverSneakers fitness membership, and all plans had $0 copays for Tier 1 drugs at in-network pharmacies. For 2027, Aetna plans may include, depending on the plan and your area:

  • A $0 monthly plan premium
  • Prescription drug coverage, with the new $2,400 yearly cap on covered drug costs
  • Dental, vision and hearing benefits
  • An Extra Benefits Card for over-the-counter items and other card-based benefits
  • A fitness membership
  • The Medicare Prescription Payment Plan to spread drug costs across the year

Benefit amounts vary widely from plan to plan, and many carriers trimmed extras for 2027. Not all plans offer all benefits, so compare the details line by line.

Aetna vs UnitedHealthcare vs Humana Medicare Advantage 2027 footprint changes and star ratings compared

Aetna vs Other Medicare Advantage Carriers in 2027

Carrier 2027 footprint change Members in 4+ star plans (2026)
Aetna About 103 counties exited More than 81%
UnitedHealthcare About 63 counties exited, about 690 plans cut About 78%
Humana About 2,400 plans cut, 39 counties added About 20%

Aetna pairs strong quality scores with a shrinking footprint. UnitedHealthcare offers the largest network. Humana is reshaping its plan lineup and has far fewer members in highly rated plans. The right carrier depends on which plans are sold in your county, whether your doctors are in network and what your drugs cost on each plan.

Aetna Medicare Advantage FAQs

➤  Is Aetna leaving Medicare Advantage in 2027?

No. Aetna is still one of the largest Medicare Advantage carriers, but it is shrinking for a second year. Analyst reviews of the 2027 CMS data show Aetna exiting about 103 counties, after closing nearly 90 plans across 34 states for 2026. If your plan is ending, you will receive a non-renewal notice and a Special Enrollment Period to choose new coverage.

➤  Does Aetna have $0 premium Medicare Advantage plans in 2027?

Aetna has historically offered a $0 premium plan in every county where it sells Medicare Advantage, and estimated that 82% of Medicare-eligible people had access to one in 2026. Availability for 2027 depends on your county, so check the plans offered at your ZIP code. You still pay your Part B premium.

➤  What star rating does Aetna Medicare Advantage have?

In the 2026 ratings, more than 81% of Aetna Medicare Advantage members were in plans rated 4 stars or higher, and more than 63% were in 4.5-star plans. CMS releases 2027 star ratings in October 2026, and about half of the measurement cut points got harder this year.

➤  Why did Aetna stop paying commissions on some Medicare Advantage plans?

Aetna told marketing organizations it will not pay broker commissions for new enrollments in 123 Medicare Advantage plans across 33 states for 2027. Carriers often do this on plans they want to shrink or may exit later. The plans are still available, but you may find fewer agents offering them.

➤  What happens if my Aetna plan is discontinued?

You will get a non-renewal notice in October. You can choose a new Medicare Advantage plan during the Annual Enrollment Period, October 15 to December 7, 2026, and you also get a Special Enrollment Period that runs through the end of February 2027. If you do nothing, you return to Original Medicare on January 1.

➤  Does Aetna offer Medicare Supplement plans?

Yes. Besides Medicare Advantage, Aetna sells Medicare Supplement (Medigap) plans, stand-alone Part D drug plans and other senior insurance products. A Medigap plan works with Original Medicare instead of replacing it.
